The 'what series is 'your name is hard to say' from' question is probably answered with Mahou Shoujo Lyrical Nanoha. In it, the main character (Takamachi Nanoha) is called, as I recall, Takamatsu Nantoka (Takamatsu Something-or-Other) by an enemy who can't remember her name.

Friendly Reminder
While I have seen a lot of magical girl anime, I still haven’t seen every single one. If your problem is directed towards one anime instead of all of them in general, I ask you to please put the anime you are talking about in parenthesis.
Example: When you’re on your real first date, but your daughter interrupts it. (Sailor Moon)
It would makes things much easier for me, and I would know if I’m using the right picture in the post.
